Subject Access Request (SAR) Form

1. Personal Information:

   • Full Name

   • Contact Information (Address, Phone Number, Email)

2. Proof of Identity:

   • Photocopy of a valid identification document (e.g., passport, driver's license)

   • Any additional documents required by the organization to verify identity

3. Details of the Request:

   • Description of the personal data you are requesting access to (if specific)

   • Any relevant time frames or periods during which the data was collected

4. Preferred Format of Response:

   • Specify whether you'd like to receive the data electronically or in hard copy

5. Declaration and Signature:

   • A statement confirming your identity and your right to access the requested information

   • Signature and date

6. Contact Details:

   • Provide an address or email where the organization can send the response.
